1. Child’s birth certificate or letter from hospital of birth (if the child is 7 years of age or older, special preparation is required).
2. Parents to be registered in our parish for at least 3 months (or provide a letter of permission from their parish and complete an interview with our pastor).
3. Catholic godparents need to be confirmed and practicing their Faith as verified on our required downloadable form, signed and officially sealed from their parish of record. 
4. The Stipend for the Sacrament:
   • • $200.00 for contributing registered families contributing $500.00 or more a year
   • • $300 for non-registered and/or parishioners contributing less than $500 a year.
   • Payments can be made below.
5. Completion of both the Online Baptismal Preparation Course for both parents and godparents and a follow up for the parents with parish priest.

• Una acta de nacimiento del niño o de la niña que será bautizado/a (si el niño es mayor de 7 años de edad hay una preparación especial para niños mayores).
• Los padres deben estar registrados en nuestra parroquia por lo menos 3 meses (o proporcionar una carta de permiso de su parroquia y completar una entrevista con el pastor).
• Los padres deben proporcionar una copia de su certificado de matrimonio católico.
• Los padrinos deben ser practicando católicos, verificado con la forma de nuestra parroquia y firmado por su párroco con el cuño oficial.
• La remuneración para el Sacramento es: Para las familias registradas que contribuyen $500.00 o más cada año $100.00. Para las familias que no están registradas o párrocos que contribuyen menos de $500.00 cada año - $150.00
• La terminacion de los: (a) Cursos de Preparacion Bautismal Online (por Internet) para los padres y padrinos y (b) la siguiente Clase de Preparación Bautismal en la parroquia.

• Must be at least 16 years of age.
• Must be a baptized Catholic who has completed the sacraments of Eucharist and Confirmation.
• May not be the parent of the child being baptized.
• Only one male sponsor or one female sponsor or one of each. If married, must be married in the  Catholic Church. They do not have to be married to one another.
• Regularly attending mass on Sunday, and living their Faith.
• A baptized non-catholic may not be a godparent but may serve as a witnessalong with a Catholic godparent, only in the event that one of the parents is not
•  Catholic Christian.
• Non-baptized persons may not serve as witnesses or godparents.
• Completion of both the Online Baptismal Preparation Course for both parents and godparents and a follow up for the parents with parish priest

Children, who have reached the age of reason (7-8yrs) , will be enrolled in Faith Formation and Religious Education classes to prepare to receive the Easter Sacrament of Baptism.  These children will also be eligible to receive their First Communion with the other children in their age level at the end of the academic year, although they may be eligible to receive the Sacrament of Baptism upon Easter. 

Adolescents, who are attending high school (14-17 yrs), who request the Sacrament of Baptism will be enrolled in Faith Formation and Religious Education classes to prepare to receive the Sacraments of Initiation at Easter. These adolescents will be enrolled in the appropriate age level classes with students preparing to receive Confirmation.
